On or about August 3, 2017, at the above addressed jobsite, employees conducting storm water retention system installation in an excavation nine (9) feet in depth did not have a safe means of egress.

29 CFR 1926.652(a)(1): Each employee in an excavation was not protected from cave-ins by an adequate protective system designed in accordance with 29 CFR 1926.652 (b) or (c). a. On or about August 3, 2017, at the above addressed jobsite, employees were conducting storm sewer retention operations in an excavation measured at nine (9) feet in depth. The excavation was not provided with cave in protection, thereby exposing the employees to cave in hazards.
