29 CFR 1926.102(a)(1): The employer did not ensure that each affected employee used appropriate eye or face protection when exposed to eye or face hazards from flying particles, molten metal, liquid chemicals, acids or caustic liquids, chemical gases or vapors, or potentially injurious light radiation:  On or about August 3, 2017, at the above addressed jobsite, an employee engaged in formwork operations was exposed to struck-by hazards when operating a pneumatic nail gun without means of eye or face protection.

29 CFR 1926.501(b)(5): Each employee on the face of formwork or reinforcing steel was not protected from falling 6 feet (1.8 m) or more to lower levels by personal fall arrest systems, safety net systems, or positioning device systems:  (1) On or about August 3, 2017, at the above addressed jobsite, employee engaged in tying rebar operations were exposed to a 12-foot fall hazard without means of tall protection.   (2) On or about August, 3, 2017, at the above addressed jobsite, employee engaged in formwork operations were exposed to a 9-foot fall hazard without means of fall protection.

29 CFR 1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed:  On or about August 3, 2017, at the above addressed jobsite, employee engageds in formwork operations were exposed to a 12-foot fall hazard when the ladder being used to gain access to the second level was not extended 3 feet from the upper landing surface.
