29 CFR 1926.416(a)(1): Employees were permitted to work in proximity to electric power circuits and were not protected against electric shock by de-energizing and grounding the circuits or effectively guarding the circuits by insulation or other means: (a) VA Medical Center - On or about July 28, 2017 employee(s) removed cover from an energized 480v panel to install new circuit breaker. Live parts were not guarded and employees were not wearing personal protective equipment for arc flash hazard.
