29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks:      (a) On or about August 3, 2017 and at time prior to that date, for the employees exposed to caught-in and/or struck-by hazards where a Jet drill press, model no. JDP-125VS-3, serial no. 186223B was operated in the wet end maintenance shop at 3630 Gateway Drive, Grand Forks, North Dakota.      (b) On or about July 28, 2017 and at time prior to that date, for the employees exposed to caught-in and/or struck-by hazards where a Exacto vertical milling machine, model no. 942B, serial no. 112939 was operated in the wet end maintenance shop at 3630 Gateway Drive, Grand Forks, North Dakota.      (c) On or about July 28, 2017 and at time prior to that date, for the employees exposed to caught-in and/or struck-by hazards where a Jet lathe, model no. GHB-1340A, serial no. 08062048A was operated in the wet end maintenance shop at 3630 Gateway Drive, Grand Forks, North Dakota.      (d) On or about July 28, 2017 and at time prior to that date, for the employees exposed to struck-by hazards where an OTC 55 ton hydraulic H-frame press was operated in the wet end maintenance shop at 3630 Gateway Drive, Grand Forks, North Dakota.      (e) On or about August 3, 2017 and at time prior to that date, for the employees exposed to caught-in and/or struck-by hazards where a Delta drill press, model no. 1, serial no. 2007 33-XC was operated in the packaging maintenance shop at 3630 Gateway Drive, Grand Forks, North Dakota.
