29 CFR 1910.215(b)(9): The distance between the abrasive wheel periphery(s) and the adjustable tongue or the end of the safety guard peripheral member at the top exceeded one fourth inch:    On or about August 3, 2017, and in times prior thereto, the tongue guard on the bench grinder, located in the shop area, was out of adjustment, exposing employees to the hazards of being struck by flying particles of the grinding stone in the event of wheel breakage.
