29 CFR 1926.451(b)(1): Each platform on all working levels of scaffolds was not fully planked or decked between the front uprights and the guardrail supports as specified in paragraphs 1926.451(b)(1)(i)-(ii).  (a) Innovation Stucco, Located at 1315 Sheridan Blvd, Lakewood, CO 80214:  The employer did not ensure that each platform on all working levels of scaffolds was fully planked or decked between the front uprights and the guardrail supports.  Employees were performing stucco work on scaffolding that was not fully planked.  This exposed employees to fall hazards.

29 CFR 1926.451(e)(1): When scaffold platforms were more than 2 feet (0.6 m) above or below a point of access, portable ladders, hook-on ladders, attachable ladders, stair towers (scaffold stairways/towers), stairway-type ladders (such as ladder stands), ramps, walkways, integral prefabricated scaffold access, or direct access from other scaffold, structure, personnel hoist, or similar surface was not used.  Crossbraces were used as a means of access.  (a)  Innovation Stucco, Located at 1315 Sheridan Ave. Lakewood, CO 80214:  The employer did not ensure that  portable ladders, hook-on ladders, attachable ladders, stair towers (scaffold stairways/towers), stairway-type ladders (such as ladder stands), ramps, walkways, integral prefabricated scaffold access, or direct access from other scaffold, structure, personnel hoist, or similar surface were used and that Crossbraces were not used as a means of access.  Employees were using the scaffolding to access the side of the building for stucco work.  This exposed employees to fall hazards.

29 CFR 1926.451(g)(1): Employees on scaffolds more than 10 feet (3.1 m) above a lower level were not protected from falling to that lower level by fall protection established in paragraphs (g)(1)(i)-(vii) of this section:  (a)  Innovation Stucco located at 1315 Sheridan Blvd, Lakewood, CO 80214:  The employer did not ensure that Employees on scaffolds more than 10 feet (3.1 m) above a lower level were protected from falling to that lower level by fall protection.  Employees were using the scaffolding to access the side of the building to perform stucco work with several areas of the scaffolding not having end rails, mid and upper rails.  This exposed employees to fall hazards.

29 CFR 1926.451(h)(1): Each employee on a scaffold was not provided with additional protection from falling hand tools, debris, and other small objects through the installation of toeboards, screens, guardrail systems, or through the erection of debris nets, catch platforms, or canopy structures that contain or deflect the falling objects.  For falling objects too large, heavy or massive to be contained or deflected, the employer did not place and secure such potential falling objects away from the edge of the surface from which they could fall.  (a) Innovation Stucco located at 1315 Sheridan Blvd. Lakewood, CO 80214:  The employer did not ensure that each employee on a scaffold was provided with additional protection from falling hand tools, debris, and other small objects through the installation of toeboards, screens, guardrail systems, or through the erection of debris nets, catch platforms, or canopy structures that contain or deflect the falling objects.  For falling objects too large, heavy or massive to be contained or deflected, the employer did not place and secure such potential falling objects away from the edge of the surface from which they could fall.  The scaffolding was being used for stucco work.  Several five gallon buckets filled with water were placed on the edge of the scaffolding.  This exposed employees to struck-by hazards.

29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b):   (a)   Innovation Stucco located at 1315 Sheridan Blvd. Lakewood, CO 80214: The employer did not ensure that employees engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were protected by guardrail systems, safety net system, or personal fall arrest system, nor were employees provided with an alternative fall protection measures.  Employees were using second and third level stairs, walkways and balconies which did not have guardrails to apply stucco to the NW walls of the building and to access scaffolding to perform stucco work.  This exposed employees to falls from height.

29 CFR 1926.1053(b)(4): Ladder(s) were used for purposes other than the purpose for which they were designed:  (a) Innovation Stucco located at 1315 Sheridan Blvd, Lakewood, CO 80214:  The employer did not ensure that a ladder used to support planking was being used for the purpose in which it was designed.  A self-supporting ladder was folded, leaning against the building with planking going through the second from top rung, supporting a worker and his tools.  This exposed employees to falls from height.

Employees were using the stairwells to access the upper level to get to scaffolding to perform stucco work.  This exposed employee to falls from height.
