29 CFR 1926.452(k)(1): Platforms of ladder jacks exceeded a height of 20 feet (6.1 m):     (a)  Jobsite:  On or about August 3, 2017, two employees were working form a ladder jack scaffold that was up to about 25 feet in height.

29 CFR 1926.501(b)(13): Each employee engaged in residential construction activities 6 feet (1.8 m)  or more above lower levels was not protected by guardrail systems, safety net systems, personal fall arrest systems, or an alternative fall protection measure under another provision of paragraph 1926.501 (b), nor did the employer demonstrate that it is infeasible or creates a greater hazard to use these systems and develop and implement a fall protection plan which meets the requirements of paragraph (k) of section 1926.502:       (a)  Jobsite:  On or about August 3, 2017, two employees who were working on a roof were not protected from falling up to about 30 feet to the ground.  The two employees were helping to replace the asphalt shingle roof.   The pitch of the roof was 10 in 12.

29 CFR 1926.451(g)(1)(i): Each employee on a boatswains' chair, catenary scaffold, float scaffold, needle beam scaffold, or ladder jack scaffold was not protected by a personal fall arrest system:  (a) Jobsite:  On or about August 3, 2017, two employees were working from a ladder jack scaffold and they were not protected from falling up to about 25 feet to the ground.  The employees were working at replacing/repairing a roof.

29 CFR 1926.503(c): When the employer has reason to believe that any affected employee who has already been trained does not have the understanding and skill required by paragraph (a) of this section, the employer is to retrain each such employee such as inadequacies in an affected employee's knowledge or use of fall protection systems or equipment indicate that the employee has not retained the requisite understanding of skill:    (a)  Jobsite:  On or about August 3, 2017, the employees who were exposed to fall hazards need to be retrained so that they retain the knowledge and skills required to minimize fall hazards while working from roofs and from scaffolding.
