29 CFR 1926.501(b)(4)(i): Each employee on walking/working surfaces was not protected from falling through holes (including skylights) more than 6 feet (1.8 m) above lower levels, by personal fall arrest systems, covers, or guardrail systems erected around each hole: a) 85 Pearl Street, New Providence, NJ 07974. Employees were exposed to fall hazards in excess of 9 feet from the walking/working platform to the concrete floor below while traversing the first floor when the stairwell opening was not protected by a guardrail system. Occurred on 08/02/2017.

Employees were exposed to fall hazards in excess of 9 feet, 6 inches when using a 2" x 12" plank as a ramp to access the inside of the new structure when the ramp had no guardrail system in place. Occurred on 08/02/2017.
