29 CFR 1910.22(a)(1): All places of employment, passageways, storerooms, and service rooms were not kept clean and orderly and in a sanitary condition. On or about August 2, 2017 and at times prior thereto, there was approximately 4 inches of dust on the electrical outlet in the vicinity of the notching machine. This condition exposed employees to fire hazards because the dust was wood based.

29 CFR 1910.132(a): Protective equipment was not used when necessary whenever hazards capable of causing injury and impairment were encountered: On or about August 2, 2017 and at times prior thereto, employees who were exposed to puncture injuries caused by protruding nails or struck by injuries caused by falling pallets were not required to wear safety shoes at the workplace. This condition exposed employees to foot injuries.
