On or about 08/02/2017, a recordable injury occurred when an employee was injured when a fresh pour column fell and hit their arm at Jobsite. The description in Column E did not identify the jobsite where the injury occurred. C. On or about 07/26/2017, a recordable injury occurred when an employee got dehydrated at Jobsite. The description in Column E did not identify the jobsite where the injury occurred. C. On or about 06/09/2017, a recordable injury occurred when an employee fell at a Jobsite, resulting in a fracture to their left ankle.
