Deficiencies noted during the inspection included, but were not limited to, the lack of a safe means of egress in an excavation over 4 feet deep and failing to provide appropriate cave in protection for an excavation determined to be at least approximately 5 feet 8 inches in depth.

29 CFR 1926.451(a)(1): Each scaffold and scaffold component was not capable of supporting, without failure, its own weight and at least 4 times the maximum intended load applied or transmitted to it:   a) Job site, 102 Riviera Drive, West View, PA: On or about August 1, 2017, during construction activities, the employer did not ensure that scaffolding was able to hold at least 4 times the maximum intended load in that the form wall/waler bracket scaffolding supports used to hold up the 2 x 6 boards were of a proper safety rating. The supports had a maximum weight rating of 125 pounds for a safe working load which was exceeded when one employee was on the scaffold.

Deficiencies noted during the inspection included but were not limited to, lack of have platforms at least 18 inches in width, scaffold components not being appropriately rated for load limits and not using a safe way to get on/off the scaffolding.
