29 CFR 1926.20(b)(2): The employer did not initiate and maintain a safety program which provides for frequent and regular inspections of jobsites, materials, and equipment to be made by a competent person (i.e., a person who is capable of identifying exists:     a.  On or about August 2, 2017, at the above addressed jobsite, an inspection was not conducted by a competent person  capable of correcting hazards such as falls, which could result in employee injuries.

29 CFR 1926.503(a)(1): The employer did not provide a training program for each employee potentially exposed to fall hazards to enable each employee to recognize the hazards of falling and the procedures to be followed in order to minimize these hazards:    a.  On or about May 17, 2017, at the above addressed jobsite, employees conducting roofing operations were not provided training on how to minimize fall protection hazards, thereby exposing employees to the hazards associated with falls.

29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b):     a.  On or about August 2, 2017, at the above addressed jobsite, employees performing residential roofing operations, fifteen (15) feet above the ground, were not protected from falls, thereby exposing employees to fall hazards.    Vayalas Roofing, Inc. was previously cited for a violation of this Occupational Safety and Health Standard 29 CFR 1926.501(b)(13), which was contained in OSHA inspection number 1224117, citation number 1, item number 1 and was affirmed as a final order on May 31, 2017, with respect to a workplace located at 3212 Cumberland Trail, Olympia Fields, IL  60461.    Vayalas Roofing, Inc. was previously cited for a violation of this Occupational Safety and Health Standard 29 CFR 1926.501(b)(13), which was contained in OSHA inspection number 1223323, citation number 1, item number 1 and was affirmed as a final order on June 6, 2017, with respect to a workplace located at 7927 Lakeside Drive, Tinley Park, IL  60487.    Vayalas Roofing, Inc. was previously cited for a violation of this Occupational Safety and Health Standard 29 CFR 1926.501(b)(13), which was contained in OSHA inspection number 1148726, citation number 1, item number 1 and was affirmed as a final order on January 3, 2017, with respect to a workplace located at 101 Monroe, Hinsdale, IL  60521.
