29 CFR 1910.147(c)(4)(ii)(D): The energy control procedures did not clearly and specifically outline the requirements for testing a machine or equipment to determine and verify the effectiveness of lockout devices, tagout devices, and other energy control measures: On or about August 3, 2017, employees were exposed to struck-by and amputation hazards when procedures were deficient for equipment such as, but not limited to, the following: a. KVAL 950 hinge prep machine/KVAL 920 strike prep machine lockout/tagout procedure does not have a verification step. b. Norfield Serial MC4071 lockout/tagout procedure says to test machine to insure it is inoperable with no specific instructions. c. Norfield Eagle Serial MC-8007 says to test machine to insure it is inoperable with no specific instructions. d. Norfield 1020 Serial TS1879, TS1278, TS1505 lockout/tagout procedure does not have a verification step. In accordance with 29 CFR 1903.19(c), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET).
