(a) In back of the building - On or about August 3, 2017 two employees were exposed to a fall hazard and potential scaffold collapse due to no mudsills being installed. These employees were working 14 feet 1 inches above the ground.

29 CFR 1926.451(f)(14): Makeshift devices, such as but not limited to boxes and barrels, shall not be used on top of scaffold platforms to increase the working level height of employees. (a) In back of the building - On or about August 3, 2017 two employees were exposed to a fall hazard due to a wooden board and siding being placed on the scaffold platform to increase the working height. The scaffolds platform height was 14 feet 1 inch high. The total fall height was 16 feet 1 inch above the ground.

29 CFR 1926.451(f)(3): Scaffolds and scaffold components shall be inspected for visible defects by a competent person before each work shift, and after any occurrence which could affect a scaffold's structural integrity. (a) In back of the building - On or about August 3, 2017 employees were exposed to a fall hazard due to the fact a scaffold 14 feet 1 inches above the ground was not inspected prior to use. This scaffold was missing some mudsills.
