29 CFR 1910.95(g)(6): The employer did not obtain a new audiogram annually for each employee exposed to noise at or above an 8 hour time weighted average exposure of 85 decibels: a) Gambone Steel - A Welder-Fitter using handheld electric grinders, drills, and metal working machines during steel fabrication was exposed to continuous noise at 66.0% of the permissible exposure level of 90 dBA or an 8 hour time weighted average of 87.0 dBA. This exposure was observed over a 480 minute sampling period. The employer did not obtain an audiogram annually for each employee exposed over the action level of 85 dBA, on or about August 4, 2017. No abatement certification or documentation required.

29 CFR 1910.95(k): The employer did not train each employee who is exposed to noise at or above an 8-hour time-weighted average of 85 decibels prior to exposure and then annually thereafter, in accordance with the requirements of paragraphs 1 and 2 under this section: a) Gambone Steel - A Laborer using handheld electric grinders, drills, and threading machine during steel fabrication was exposed to continuous noise at 71.9% of the permissible exposure level of 90 dBA or an 8 hour time weighted average of 87.6 dBA. This exposure was observed over a 480 minute sampling period. The employer did not train employees prior to exposures over the action level of 85 dBA, on or about September 8, 2017. b) Gambone Steel - A Welder-Fitter using handheld electric grinders, drills, and metal working machines during steel fabrication was exposed to continuous noise at 66.0% of the permissible exposure level of 90 dBA or an 8 hour time weighted average of 87.0 dBA. This exposure was observed over a 480 minute sampling period. The employer did not annually train employees exposed over the action level of 85 dBA, on or about September 8, 2017. No abatement certification or documentation required.
