(a)       On or about August 7, 2017, the employer failed to submit documentation and certification of corrective action for citation #1, item #1a issued on December 11, 2017 with an abatement due date of December 21, 2017.   (b)       On or about August 7, 2017, the employer failed to submit documentation and certification of corrective action for citation #1, item #1b issued on December 11, 2017 with an abatement due date of December 21, 2017.   (c)       On or about August 7, 2017, the employer failed to submit documentation and certification of corrective action for citation #1, item #1c issued on December 11, 2017 with an abatement due date of December 21, 2017.

29 CFR 1926.451(b)(1): Each platform on all working levels of scaffolds was not fully planked or decked between the front uprights and the guardrail supports as specified in paragraphs 1926.451(b)(1)(i)-(ii)  On or about August 7, 2017 at 2171 LaVista Road, Atlanta, Georgia, where employees were grouting the cells in a masonry, the employees were exposed to a fall of approximately 12 feet to the lower level.  The working level was not fully decked.

29 CFR 1926.451(e)(1): When scaffold platforms were more than 2 feet (0.6 m) above or below a point of access, portable ladders, hook-on ladders, attachable ladders, stair towers (scaffold stairways/towers), stairway-type ladders (such as ladder stands), ramps, walkways, integral prefabricated scaffold access, or direct access from other scaffold, structure, personnel hoist, or similar surface was not used.  Crossbraces were used as a means of access.  On or about August 7, 2017 at 2171 LaVista Road, Atlanta, Georgia, two employees were grouting the cells in a masonry wall, working 12 feet above the lower level.  The employees were exposed to a fall hazard when they had to climb the scaffold frame to access/egress the working level.

29 CFR 1926.451(g)(1)(v): Each employee on a walkway located within a scaffold was not protected by a guardrail system (with a minimum 200 pound toprail capacity) installed within 9.5 inches (24.1 cm) of and along at least one side of the walkway.   On or about August 7, 2017 at 2171 LaVista Road, Atlanta, employees building a masonry wall were exposed to a fall hazard of approximately 12 feet to the lower level. There were no guard rails installed to prevent the employees from falling.
