29 CFR 1910.1001(j)(3)(i): Building and facility owners shall determine the presence, location, and quantity of ACM and/or PACM at the work site. Employers and building and facility owners shall exercise due diligence in complying with these requirements to inform employers and employees about the presence and location of ACM and PACM. a) On or about August 7, 2017, the employer was not aware of building surfaces were asbestos containing material (ACM) or presumed asbestos containing material (PACM). The 9 by 9 inch floor tile in several areas of the facility is PACM.

29 CFR 1910.1001(j)(7)(iv): The employer shall also provide, at no cost to employees who perform housekeeping operations in an area which contains ACM or PACM, an asbestos awareness training course, which shall at a minimum contain the following elements: health effects of asbestos, locations of ACM and PACM in the building/facility, recognition of ACM and PACM damage and deterioration, requirements in this standard relating to housekeeping, and proper response to fiber release episodes, to all employees who perform housekeeping work in areas where ACM and/or PACM is present. Each such employee shall be so trained at least once a year. a) Employees maintaining 9 inch by 9 inch floor tiling in the office area were not notified that the floor tiling was presumed asbestos containing material and had not received the required asbestos awareness training described by this standard.

Every enclosed workplace shall be so constructed, equipped, and maintained, so far as reasonably practicable, as to prevent the entrance or harborage of rodents, insects, and other vermin. A continuing and effective extermination program shall be instituted where their presence is detected. a) On or about August 7, 2017, mouse feces were observed behind the trash can in the employee break area.
