29 CFR 1926.501(b)(1): Each employee on a walking/working surface with an unprotected side or edge which was 6 feet (1.8 m) or more above a lower level was not protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest systems. On or about August 4, 2017, in the sub division at 3658 Martin Farm Road, Suwanee, Georgia, on lot#6, employees had stored their materials (lumber used in the foundation application) on the edge of an excavation. When retrieving materials, the employees were exposed to a fall of approximately 7 feet to the lower level.

29 CFR 1926.652(a)(1): Each employee in an excavation was not protected from cave-ins by an adequate protective system designed in accordance with 29 CFR 1926.652 (b) or (c). On or about August 4, 2017 at Lot#6, employees installing form work for that placement of the foundation and basement slab were exposed to a cave in hazard working beneath an 7 foot vertical wall in an unprotected trench.

29 CFR 1926.701(b): All protruding reinforcing steel, onto and/or into which employees could fall or come against, was not guarded to eliminate the hazard of impalement: On or about August 4, 2017, on the subdivision at 3658 Martin Farm Road, Suwanee, Georgia at Lot #6, where employees were installing formwork for the footings for the home under construction, employees were exposed to an impalement hazard when the protruding reinforcing steel was not covered.
