29 CFR 1926.451(g)(1): Each employee on a scaffold more than 10 feet (3.1 m) above a lower level was not protected from falling to that lower level:  a) 86 Farmington Ave., Longmeadow, MA: On or about  August 8, 2017, an employee was observed working from a ladder jack scaffold platform, more than 16 feet above the ground below without any fall protection.  Specific abatement documentation that this violation has been corrected must be provided within 10 days of the abatement in accordance with 29 CFR 1903.19(d)(1).

Specific abatement documentation that this violation has been corrected must be provided within 10 days of the abatement in accordance with 29 CFR 1903.19(d)(1).

29 CFR 1926.1053(b)(1): When portable ladders are used for access to an upper landing surface, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface to which the ladder is used to gain access:  a) 86 Farmington Ave., Longmeadow, MA: On or about  August 8, 2017, employees were observed climbing a portable extension ladder to access the roof top work location whose side rails did not extend 3 feet past the upper landing (roof) surface.  Specific abatement documentation that this violation has been corrected must be provided within 10 days of the abatement in accordance with 29 CFR 1903.19(d)(1).
