29 CFR 1926.451(g)(1)(i): Each employee on a boatswains' chair, catenary scaffold, float scaffold, needle beam scaffold, or ladder jack scaffold was not protected by a personal fall arrest system:      a) On or about 08/07/17, on a jobsite at a house on 941 Prospect  Avenue,  Buffalo, NY:  Employees  were  exposed  to a potential fall of approx.  20 ft. from  the pik on a Ladder Jack Scaffold erected on the right side of the house while doing roof tear-off work at the roof edge.  No Fall protection system was being utilized.       NO ABATEMENT CERTIFICATION REQUIRED

29 CFR 1926.452(k)(5): Scaffold platforms were bridged on to another:   a) On or about 08/07/17, on a jobsite at a house on 941 Prospect  Avenue,  Buffalo, NY:  Employees were exposed  to a potential fall of approx.  20 ft. from  the aluminum piks on a Ladder Jack Scaffold.  Aluminum piks were overlapped (bridged) on the Ladder Jack Scaffolds erected on both sides of the house.    NO ABATEMENT CERTIFICATION REQUIRED

29 CFR 1926.501(b)(11): Each employee on a steep roof with unprotected sides and edges 6 feet (1.8 m) or more above lower levels was not protected from falling by guardrail systems with toeboards, safety net systems, or personal fall arrest systems:       a) On or about 08/07/17, on a jobsite at a house on 941 Prospect  Avenue, Buffalo, NY:  Employees engaged in roofing tear-off work on a steep roof on the right side of the house were exposed to a fall of approx.  22 foot.  No fall protection system was being utilized. Pitch of the roof was approx. 12 in 12.        NO ABATEMENT CERTIFICATION REQUIRED

29 CFR 1926.21(b)(2): The employer did not instruct each employee in the recognition and avoidance of unsafe conditions and the regulations applicable to his/her environment to control or eliminate any hazards or other exposure to illness or injury:   a) On or about 08/07/17, on a jobsite at a house on 941 Prospect Avenue, Buffalo, NY:  Employees engaged in construction work including roofing & clean-up were not trained to recognize potential hazards on this type of jobsite, including but not limited to falls from roofs, ladders, skylights, scaffolds, electrical hazards, unguarded tools/machines, fire hazards, and the need for personal protective equipment to protect them from various hazards. They also were not trained in the applicable OSHA regulations to control or eliminate these hazards.  ABATEMENT CERTIFICATION REQUIRED

29 CFR 1926.100(a): Employees working in areas where there was a possible danger of head injury from impact, or falling or flying objects, or from electrical shock and burns, were not protected by protective helmets:     a) On or about 08/07/17, on a jobsite at a house on 941 Prospect  Avenue,  Buffalo, NY:  An employee working on the ground doing clean-up work in the vicinity of employees working  above on the roof and from  a Ladder Jack Scaffold  was exposed to the potential hazard of being struck by materials/tools that could fall off the roof/scaffold. The employee was not wearing a hardhat.       NO ABATEMENT CERTIFICATION REQUIRED
