29 CFR 1910.28(b)(8):  For repair pits, service pits, or assembly pits that were less than 10 feet (3 m) deep when fall protection systems were not required, the employer did not comply with provisions (i)-(iii) of this section:    (a) Grease Monkey #702: On or about August 8, 2017, and at times prior, employees were exposed to fall hazards in excess of 4 feet into grease pits in that bay number three was left open with the net not in use.     Abatement Note:  Abatement certification and documentation are required for this item (see enclosed "Certification of Corrective Action Worksheet").    Abatement Note:  In addition to the provisions listed in this section, acceptable abatement includes the use of conventional fall protection (such as guardrails) or safety net systems used in accordance with 29 CFR 1926 Subpart M.

29 CFR 1910.134(c)(2)(i): Respirator users were not provided with the information contained in Appendix D to 29 CFR 1910.134 when the employer determined that any voluntary respirator use was permissible:  (a) Grease Monkey #702: On or about August 8, 2017, and at times prior, employees were wearing a filtering face-piece device for comfort purposes while not being provided information contained in 29 CFR 1910.134, Appendix D.
