29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks: On or about 8/8/2017, the employer failed to ensure that machinery including, but not limited to the Bolt Latch #293, Bracket #233, Adapter #291, and the Tschudin #252 were provided with interlocked doors on the sides of the machinery. The doors were able to be opened while the machines were in operation.

29 CFR 1910.147(c)(4)(i): Procedures were not developed, documented and utilized for the control of potentially hazardous energy when employees were engaged in activities covered by this section: On or about 8/8/2017, the employer failed to ensure that machinery including, but not limited to the Bolt Latch #293, Bracket #233, Adapter #291, and the Tschudin #252 were provided with interlocked doors on the sides of the machinery. Employees were tasked with opening the side doors to utilize compressed air to clean off debris on the tooling. The machine was not locked out or provided interlocks on the side doors.
