29 CFR 1910.37(a)(3): Exit routes were not kept free and unobstructed: (a) WestRock Services, Inc., at 100 E. Benson Rd., Sioux Falls SD, 57104: On and prior to August 11, 2017, the employer did not ensure that exit routes were kept free and unobstructed in that emergency exits particularly in the northwest and northeast areas of the plant were blocked with equipment and materials. This exposed employees to hazards associated with fires, smoke inhalation, and other emergencies.

29 CFR 1910.176(a): Use of mechanical equipment. Where mechanical handling equipment is used, sufficient safe clearances shall be allowed for aisles, at loading docks, through doorways and wherever turns or passage must be made. Aisles and passageways shall be kept clear and in good repair, with no obstruction across or in aisles that could create a hazard: a)WestRock Services, Inc., at 100 E. Benson Rd., Sioux Falls SD, 57104: On and before August 11, 2017, the employer did not ensure that aisles and passageways were kept clear of obstructions n the area where paper rolls were stored. This allowed for Powered Industrial Trucks (PITs) to come into contact with the material causing damage to the PIT's as well as potential for material to collapse. This condition exposed employees to the hazard of being crushed and/or caught in-between due to shifting or collapsing paper rolls and stacked material.

Benson Rd., Sioux Falls SD, 57104: On and before August 11, 2017, the employer did not ensure that employees were protected from a material/product collapse while working near loads of unsecured materials being transported via forklift. This condition exposed employees to struck-by hazards.
