29 CFR 1910.147(f)(1)(v): All systems were not de-energized and energy control measures reapplied in accordance with paragraph (d) of this section to continue the servicing and/or maintenance: a) Radius Department, 250 S.R. 61 South, Schuylkill Haven, PA - Employees performing maintenance/testing of circuit boards within an electrically energized control cabinet physically left the area and worked in a different department without de-energizing the control cabinet thereby exposing employees to a contact with an electrical current hazard, as observed on August 8, 2017. In accordance with 29 CFR 1903.19, abatement certification is not required for this violation.

29 CFR 1910.303(g)(2)(i): Except as elsewhere required or permitted by Subpart S of Part 1910, live parts of electric equipment operating at 50 volts or more were not guarded against accidental contact by use of approved cabinets or other forms of approved enclosures or by any of the means identified in paragraphs (A), (B), (C), and (D) of 29 CFR 1910.303(g)(2)(i): a) Radius Department, 250 S.R. 61 South, Schuylkill Haven, PA - Employees operated the COE Press with electrical controls on top of an open control cabinet exposing employees to a contact with an electrical current hazard, on or before August 8, 2017. In accordance with 29 CFR 1903.19, abatement certification is required for this violation.
