This was most recently documented at 25201 E. 78 Hwy, Independence, Missouri.  The employer's process hazard analysis did not address the effect of loss of video surveillance on the lead styphnate manufacturing process.

29 CFR 1910.119(f)(1)(i)(D): Written operating procedures providing clear instruction for conducting operations in a covered process safely and consistently with process safety information addressing emergency shutdown, including the conditions under which emergency shutdown is required, and the assignment of shutdown responsibility to qualified operators to ensure that emergency shutdown is executed in a safe and timely manner have not been developed:    The employer is failing to protect employees from explosion and fire hazards by failing to provide emergency shutdown procedures in the lead styphnate operating procedure for the lead styphnate manufacturing process. This was most recently documented at 25201 E. 78 Hwy, Independence, Missouri.  The company developed a lead styphnate Standard Operating Procedure, which does not contain procedures for emergency shutdown when adding the first material, magnesium styphnate, to a batch.

29 CFR 1910.119(f)(1)(ii)(A): The employer did not develop and implement written operating procedures that addressed the consequences of deviations from the operating limits in the covered process;      The employer is failing to protect employees from explosion and fire hazards by failing to provide clear instructions (Standard Operating Procedures) in case of deviation from normal operating conditions in the lead styphnate operating procedure for the lead styphnate manufacturing process.  This was most recently documented at 25201 E. 78 Hwy, Independence, Missouri.  The company developed a lead styphnate Standard Operating Procedure, which does not address deviation from normal operating/manufacturing conditions at the beginning of the manufacturing process.  Abort procedures only address shutdown after the addition of lead nitrate, the second of two ingredients added to the process.  The SOP does not include a physical inspection of process equipment such as but not limited to the boats, to detect the presence of material such as magnesium styphnate, after aborting the process.

This was most recently documented at 25201 E. 78 Hwy, Independence, Missouri, regrading the following equipment:    a)    Written procedures to ensure mechanical integrity of equipment were not established or implemented for the lead nitrate valve in the lead styphnate manufacturing process.      b)    Written procedures to ensure mechanical integrity of equipment were not established or implemented for the magnesium styphnate valve in the lead styphnate manufacturing process.    c)     Written procedures to ensure mechanical integrity of equipment were not established or implemented for the Delta V Control System Monitor and associated transmitting equipment, monitoring equipment for the lead styphnate manufacturing process.
