(i) Except as provided elsewhere in this section, the employer must ensure that each employee on a walking-working surface with an unprotected side or edge that is 4 feet (1.2 m) or more above a lower level is protected from falling by one or more of the following: 29 CFR 1910.28(b)(1)(i)(A): Guardrail systems or in the alternative 29 CFR 1910.28(b)(1)(i)(B): Safety net systems; or or in the alternative 29 CFR 1910.28(b)(1)(i)(C): Personal fall protection systems, such as personal fall arrest, travel restraint, or positioning systems. a. On or before August 9, 2017, where employees worked from the roofs of recreational vehicles in the service area, the employees were not protected from falls, thereby being exposed to falls from heights of up to 13 feet.

29 CFR 1910.178(a)(4): Modifications and additions which affect capacity and safe operation shall not be performed by the customer or user without manufacturers prior written approval. Capacity, operation, and maintenance instruction plates, tags, or decals shall be changed accordingly. a. On or before August 9, 2017, where employees operated powered industrial vehicles that had been modified with shop made attachments to move recreational vehicles around the facility, the employer failed to obtain written approval for the attachments and the maintenance instruction plates, tags, or decals for the powered industrial vehicles had not been changed, thereby exposing employees to struck-by or other hazards.
