29 CFR 1926.453(b)(2)(v): A body belt was not worn and/or a lanyard was not attached to the boom or basket when working from an aerial lift: On or about August 9, 2017, at the above addressed site, an employee was exposed to a fall hazard of approximately 12 feet 6 inches when performing paint work from the basket of an aerial lift while not wearing a harness with a lanyard attached to the boom or basket of that lift.

29 CFR 1910.1200(g)(8): The employer did not maintain in the workplace copies of the required safety data sheets for each hazardous chemical, and did not ensure that they were readily accessible during each work shift to employees when they were in their work area(s): On or about August 9, 2017, at the above addressed site, employees were using chemicals including but not limited to Sherwin-Williams; Super Paint Exterior Acrylic Latex Paint, Loxon Acrylic Conditioner, and Masonry Coatings Systems while the employer did not maintain the safety data sheets for those products at the worksite.
