29 CFR 1910.147(c)(4)(i): Procedures were not developed, documented and utilized for the control of potentially hazardous energy when employees were engaged in activities covered by this section:  a)  At Laser Weld Machine:  On or about August  8, 2017 and at times prior; employees were exposed to struck-by hazards in that the employer failed to ensure employees utilized the hazardous energy control procedures when employees were engaged in servicing and maintenance activities, including but not limited to, removing parts and making repairs to the laser weld machine.

29 CFR 1910.147(c)(4)(ii): Procedures did not clearly and specifically outline the scope, purpose, authorization, rules, and techniques to be utilized for the control of hazardous energy, and the means to enforce compliance including, but not limited to, 29 CFR 1910.147(c)(4)(ii)(A), (c)(4)(ii)(B), (c)(4)(ii)(C) and (c)(4)(ii)(D):    a)  At Upper Rail Line:  On or about September 27, 2017 and at times prior; the employer exposed his employees to hazardous energy in that employees were allowed to work on and around equipment that was locked out using energy control procedures that did not contain the required elements directed by the standard.

29 CFR 1910.147(c)(7)(i)(A): Authorized employee(s) did not receive training in the recognition of applicable hazardous energy sources, the type and magnitude of the energy available in the workplace, and the methods and means necessary for energy isolation:  a)  At Laser Weld Machine:  On or about August  8, 2017 and at times prior; the employer allowed the first shift team leader to perform routine maintenance on the Laser Welder without first ensuring the employee was trained as an authorized employee exposing the employee struck-by hazards and electrical hazards.

29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks:  a)  At the FKK Mail Line:  On or about August September 17, 2017 and at times prior; the employer allowed employees to perform routine maintenance on station 6 without first ensuring guards were in place exposing employees to struck-by hazards.  An employee performing routine maintenance on station 6 was struck-by the machine in an area that was not guarded.
