29 CFR 1910.147(c)(1): The employer did not establish a program consisting of an energy control procedure, employee training and periodic inspections to ensure that before any employee performed any servicing or maintenance on a machine or equipment where the unexpected energizing, startup or release of stored energy could occur and cause injury, the machine or equipment shall be isolated from the energy source and rendered inoperative:    On or about July 25, 2017, the employer did not establish a program to protect employees involved with servicing and maintenance of machines and equipment from the unexpected energizing, start up, or release of stored energy.  Energy sources included, but are not limited to, electrical and hydraulic. Employees are tasked with maintaining, servicing, and operating equipment including but not limited to the conveyor saw, chop saw, and stripper/dismantler.

Employees tasked with the LO/TO of equipment for maintaining and servicing had not received authorized training. Employees tasked with operating and working near equipment had not received affected training. Energy sources included, but are not limited to, electrical and hydraulic. 29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks:    On or about July 25, 2017, the employer failed to ensure that pallet stripper/dismantler band saw was guarded effectively to prevent employee contact with the blade. The table was not long enough to prevent employees from reaching the blade.
