29 CFR 1910.178(a)(4): Modifications or additions which affect capacity and safe operation of powered industrial truck were performed by the employer without the manufacturer's prior written approval. Fabrication Shop - The employer modified the Yale powered industrial truck by adding fork extensions to the original forks without receiving the manufacturer's prior written approval, and a capacity plate that includes the extension was not on the truck.
