Deficiencies noted during the inspection included, but were not limited to, not installing appropriate braces on pump jack scaffolding, failing to use fall protection during scaffold erection activities, lack of appropriate scaffold footing and not ensuring that a safe means of scaffold access existed during scaffold erection activities.
