29 CFR 1904.39(a)(2): An oral or written report of a work related incident which resulted in an in-patient hospitalization of one or more employees or an employee's amputation or an employee's loss of an eye, was not reported to the Occupational Safety & Health Administration (OSHA) within 24 hours after the occurrence: a- Milano Granite & Marble, 3521 Victory Blvd, Staten Island, NY - Throughout Facility & Worksites - Notification of a work related incident which occurred on 6/26/2017 which resulted in an amputation was not made by the employer to OSHA within 24 hours after the occurrence.
