On August 9, 2017, the Chicago K & D mechanical full revolution press brake used with a unitized tool as a power press did not have a guard to cover the opening above the tool and below the ram, exposing employees to an amputation hazard.

On August 9, 2017, in both shops, there were oxygen cylinders stored near methane and acetylene cylinders, exposing employees to a fire hazard.
