When unitized tooling was used, the opening between the top of the punch holder and the face of the slide, or striking pad, was not safeguarded in accordance with the requirements of paragraph (c) of this section:  a. On August 9, 2017, the Chicago K & D mechanical full revolution press brake used with a unitized tool as a power press did not have a guard to cover the opening above the tool and below the ram, exposing employees to an amputation hazard.

29 CFR 1910.253(b)(4)(iii): Oxygen cylinders in storage were not separated from fuel-gas cylinders or combustible materials (especially oil or grease), a minimum distance of 20 feet (6.1 m) or by a noncombustible barrier at least 5 feet (1.5 m) high having a fire-resistance rating of at least one-half hour:  a. On August 9, 2017, in both shops, there were oxygen cylinders stored near methane and acetylene cylinders, exposing employees to a fire hazard.
