29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b):  On or about August 10, 2017, at the above addressed site, employees were exposed to a fall hazard of approximately 11 feet 4 inches when performing plywood sheathing installation work on a residential building under construction while not being protected from falling to the lower level.

29 CFR 1926.503(a)(1): The employer did not provide a training program for each employee potentially exposed to fall hazards to enable each employee to recognize the hazards of falling and the procedures to be followed in order to minimize these hazards:   On or about August 10, 2017, at the above addressed site, employees were exposed to a fall hazard when installing plywood sheathing to the roof of a residence under construction prior to being provided with a training program that would enable them to recognize the hazards associated with falling.

29 CFR 1926.403(b)(2): Listed, labeled, or certified equipment was not installed and used in accordance with instructions included in the listing, labeling, or certification:   On or about August 10, 2017, at the above addressed site, employees were exposed to an electrical shock hazard when using a 4 by 4 inch metal outlet box with flexible electrical wiring as an extension cord to power carpentry hand tools and an air compressor which was not in accordance with the manufactures instructions.
