29 CFR 1926.102(a)(1): The employer did not ensure that each affected employee uses appropriate eye or face protection when exposed to eye or face hazards from flying particles, molten metal, liquid chemicals, acids or caustic liquids, chemical gases or vapors, or potentially injurious light radiation: On or about August 9, 2017 at a worksite located at 4309 West 49th Street, Cleveland, OH 44144, during residential roofing work, the employer did not ensure employees were protected from eye hazards by ensuring eye protection was available and worn while operating circular saws and pneumatic nail guns at approximately 120 psi.

29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b): On or about August 9, 2017, at a worksite located at 4309 West 49th Street, Cleveland, OH 44144, during residential roofing work, employees were exposed to fall heights of approximately 13 feet. The employer did not ensure employees were protected from fall hazards by ensuring a fall protection system/device was being used.
