29 CFR 1910.151(c): Where the eyes or body of any person may be exposed to injurious corrosive materials, suitable facilities for quick drenching or flushing of the eyes and body were not provided within the work area for immediate emergency use: a) Workshop, on or about August 10, 2017- An eye wash station was not located within the workshop for quick drenching or flushing of the eyes for emergency use. Employees poured cement mixture in molds and smoothed the tops where splashes potential hazards existed.

Employees used and were exposed to but not limited to the following: MSP Odorless Release Agent, a combustible liquid, which contained aliphatic hydrocarbon and petroleum hydrocarbon; and Portland Cement, which contained gypsum, iron oxide, calcium carbonate, calcium oxide and crystalline silica (0-5%).
