29 CFR 1926.451(e)(1): When scaffold platforms were more than 2 feet (0.6 m) above or below a point of access, portable ladders, hook-on ladders, attachable ladders, stair towers (scaffold stairways/towers), stairway-type ladders (such as ladder stands), ramps, walkways, integral prefabricated scaffold access, or direct access from other scaffold, structure, personnel hoist, or similar surface was not used: A) South west end of building, on or about August 10, 2017, employees accessed the second and third tier of a three tiers high scaffold by climbing the support legs of the scaffold, exposing the employees to a fall hazard of up to 14 feet.

29 CFR 1926.451(g)(4)(i): Guardrail systems were not installed along all open sides and ends of platforms before being released for use by employees other than erection/dismantling crews: A) West side of building, on or about August 10, 2017, employees were working from a welded frame scaffold that was missing a mid-rail on the third level platform, exposing the employee to a 14 foot fall hazard. B) North west end of building, on or about August 10, 2017, employees were working from an outrigger on a welded frame scaffold that did not have guardrails on the end of the outrigger, exposing the employee to a 10 1/2 foot fall hazard.

29 CFR 1926.602(c)(1)(vi): Industrial truck(s) in use did not meet the applicable requirements of design, construction, stability, inspection, testing, maintenance, and operation, as defined in American National Standards Institute B56.1-1969, Safety Standards for Powered Industrial Trucks: A) North west side of building, on or about August 10, 2017, a JCB rough terrain forklift with a telescopic boom was left unattended while in a raised position with a load on the forks, exposing employees to a struck by hazard or a destabilized scaffold if the load fell.
