29 CFR 1926.451(g)(4)(i): Guardrail systems were not installed along all open sides and ends of platforms before being released for use by employees other than erection/dismantling crews: A) West side of building, on or about August 10, 2017, employees were working from a welded frame scaffold that was missing a mid-rail on the third level platform, exposing the employee to a 14 foot fall hazard. B) North west side of building, on or about August 10, 2017, employees were working from an outrigger on a welded frame scaffold that did not have guardrails on the end of the outrigger, exposing the employee to a 10 1/2 foot fall hazard.
