29 CFR 1926.451(g)(4)(i): Guardrail systems were not installed along all open sides and ends of platforms before being released for use by employees other than erection/dismantling crews:      Site: 832 Lexington Ave Brooklyn, NY    On or about 8/9/17    a) An employee worked from a tubular welded frame scaffold, he was exposed to falling approximately 60 feet. The employee was not using fall protection and the scaffold was not equipped with endrails.
