29 CFR 1926.20(b)(2): The employer did not initiate and maintain a safety program which provides for frequent and regular inspections of jobsites, materials, and equipment to be made by a competent person. Location: 127 Kimball Road, Amesbury, MA On or about August 10, 2017, the employer did not perform a frequent and regular inspection of the job site to protect sub-contract workers from the following but not limited to hazards; 1. Fall hazards greater than 22-feet, while working from the third floor, second floor, and rear balcony without fall protection; 2. Fall hazards greater than 16-feet, 3-inches, while working from the second and third floor with un-guarded window openings; 3. Fall hazards greater than 4-feet, while accessing and exiting the house without a staircase; 4. Eye injury hazards from workers operating pneumatic nail guns and a circular saw without safety glasses; 5. Housekeeping hazards due to debris, trash, and protruding nails that was not removed from the work area.
