29 CFR 1926.403(b)(2): Listed, labeled, or certified equipment was not installed and used in accordance with instructions included in the listing, labeling, or certification: a) jobsite at 2812 Shadow Drive in Sharpsville, PA. On or about August 10, 2017, a metal, knock-out electrical box was attached to the end of an extension cord. The cord and outlet box was used to power the saws and other electrical tools. The metal box was not installed or used in accordance with the listing and/or labeling instructions.

29 CFR 1926.416(e)(1): Worn or frayed electric cords or cables were used: a) jobsite at 2812 Shadow Drive in Sharpsville, PA. On or about August 10, 2017, the extension cord, used to power saws and other electrical tools, was damaged in that, the outer sheathing was sliced open, exposing the inner wires of the cord.

29 CFR 1926.501(b)(6): Each employee on ramps, runways, and other walkways were not protected from falling 6 feet (1.8 m) or more to lower levels by guardrail systems: a) jobsite at 2812 Shadow Drive in Sharpsville, PA. On or about August 10, 2017, the ramp to enter the structure at the front of the property was not protected by a guardrail system. Employees entering the structure from the ramp were exposed to a fall of at least 6 feet.
