29 CFR 1926.451(b)(5)(ii): The end of a platform greater than 10 feet in length extended over its support more than 18 inches and the platform was not designed and installed so that the cantilevered portion of the platform is able to support employees without tipping, or has guardrails which block employee access to the cantilivered end: a) Exterior Rear - An employee was walking on a pumpjack scaffold work platform that extended over 10 feet beyond the support, on or about 08/08/2017. In accordance with 29 CFR 1903.19(d), abatement certification is required for this violation (using the Certification of Corrective Action Worksheet).

29 CFR 1926.451(g)(1): Employees on scaffolds more than 10 feet (3.1 m) above a lower level were not protected from falling to that lower level by fall protection established in paragraphs (g)(1)(i)-(vii) of this section: a) Exterior Rear - An employee was observed exiting the rear 2nd floor door and stepping onto the platform of a pumpjack scaffold, 13 feet, 5 inches, above the ground and not using fall protection, on or about 08/08/2017. In accordance with 29 CFR 1903.19(d), abatement certification is required for this violation (using the Certification of Corrective Action Worksheet).
