29 CFR 1910.95(g)(6): At least annually after obtaining the baseline audiogram, the employer did not obtain a new audiogram for each employee exposed at or above an 8-hour time-weighted average of 85 decibels: a) Concrete block plant, on or about 8/22/17: An employee engaged in the manufacturing of concrete block was exposed to noise from machinery including, but not limited to, the concrete block machine at a dose of 173.6 % or a noise level of 94.1 decibels (dBA), based on an 8-hour time weighted average (TWA), during the 201 minute sampling period. A zero increment was included for the 279 minutes not sampled. No annual audiogram was provided within a 12 month period since the last one was on 6/24/16. Abatement certification must be submitted for tis item.

29 CFR 1910.147(c)(6): The employer did not conduct a periodic inspection of the energy control procedure at least annually to ensure that the procedure and the requirements of this standard are being followed. a) Concrete block area, on or about 8/21/17: A periodic inspection of equipment with multiple energy sources was not done for equipment in the facility including, but not limited to, the mixer which operates on electrical energy and pneumatic energy, and the cuber which operates on electrical energy and hydraulic energy. The periodic inspection of lockout/tagout procedures must: - be performed by an authorized employee other than the ones utilizing the energy control procedure being inspected. - be conducted to correct any deviations or inadequacies identified. - include a review, between the inspector and each authorized employee, of that employee's responsibilities under the energy control procedure being inspected. - include a review between the inspector and each authorized and affected employee, of that employee's responsibilities under the energy control procedure being inspected, and the elements set forth in paragraph (c)(7)(ii) of this section. Abatement certification must be submitted for this item.
