29 CFR 1926.100(a): Employees working in areas where there was a possible danger of head injury from impact, or falling or flying objects, or from electrical shock and burns, were not protected by protective helmets:  (a). Worksite located at 3309 N. Baltimore Road, Kirksville, Missouri- Employee was observed working and walking directly beneath elevated employees and materials on an adjustable scaffold loaded with masonry blocks and was not wearing a hard hat while working beneath the materials.

29 CFR 1926.102(a)(1): The employer shall ensure that each affected employee uses appropriate eye or face protection when exposed to eye or face hazards from flying particles, molten metal, liquid chemicals, acids or caustic liquids, chemical gases or vapors, or potentially injurious light radiation:   (a). Worksite located at 3309 N. Baltimore St., Kirksville, Missouri- Employees were observed using radial chop saws to cut masonry blocks and were not protected from impact by  the use of any means of eye protection.

29 CFR 1926.451(c)(1)(ii): Guys, ties, and/or braces were not installed  according to the scaffold manufacturer's recommendations or at the closest horizontal member to the 4:1 height and repeated vertically at locations of horizontal members every 20 feet (56.1 m) or less thereafter for scaffolds 3 feet ((0.91 m) wide or less, and every 26 feet (7.9 m) or less therafter for scaffolds greater than 3 feet (0.91m) wide:  (a). Worksite located at 3309 N. Baltimore St., Kirksville, Missouri- Employees were exposed to a fall or scaffold collapse hazards in that the tower scaffold being used on the site was more than 20' high and was not tied into the structure

29 CFR 1926.451(g)(4)(i): Guardrail systems were not installed along all open sides and ends of platforms:   (a). Worksite located at 3309 N. Baltimore St., Kirksville, Missouri- Employees were exposed to fall hazards in that employees were working at the ends of the scaffold and were not protected from falling by the use of an adequate end rails to prevent them from walking off the ends of the scaffolds.

29 CFR 1926.451(g)(4)(iii): When midrails, screens, mesh, intermediate vertical members, solid panels, or equivalent structural members are used, they were not installed between the top edge of the guardrail system and the scaffold platform:   (a). Worksite located at 3309 N. Baltimore Road, Kirksville, Missouri- Employees were exposed to fall hazards in that the employer failed to protect employees from falling by not installing midrails on the working platforms of  the scaffold system.
