29 CFR 1910.1052(d)(1)(i): Where methylene chloride was present in the workplace, the employer did not determine each employee's exposure: Location - Assembly 1 and 2 On or about August 10, 2017, employees were exposed to methylene chloride while using hazardous commercial glue, such as Weld On #3, for final assembly of plastic products. Weld On #3 contains approximately 80 to 95% methylene chloride, and employer did not perform exposure monitoring for methylene chloride to determine each affected employee's exposure.

29 CFR 1910.1200(h)(1): Employees were not provided effective information and training on hazardous chemicals in their work area at the time of their initial assignment and whenever a new hazard that the employees had not been previously trained about was introduced into their work area: Location - Assembly 1 and 2 On or about August 10, 2017, employer did not provide effective information and training for employees required to use hazardous commercial glue, such as Weld On #3 and Weld On 717 (contain Methyl Chloride, Methyl Methacrylate monomer, Tetrahydrofuran, and Methyl Ethyl Ketone), for final assembly of plastic products.
