29 CFR 1926.100(a):  Employees working in areas where there was a possible danger of head injury from impact, or falling or flying objects, or from electrical shock and burns, were not protected by protective helmets:    (a)   On or about August 11, 2017 at the location 2718 Beltline Road, Sunnyvale, TX, at least two (2) employees working from the ground passing materials to employees working on the roof were exposed to head injuries where overhead hazard exists.

29 CFR 1926.102(a)(1): Employees were not provided with eye and face protection equipment when machines or operations present potential eye or face injury from physical, chemical, or radiation agents:    (a)   On or about August 11, 2017 at the location 2718 Beltline Road, Sunnyvale, TX, employees were performing roofing operations while using nail guns. Employees were not wearing safety glasses and were exposed to eye injuries.

29 CFR 1926.501(b)(13): Each employee engaged in residential construction activities 6 feet or more above lower levels was not protected by guardrail systems, safety net systems, or personal fall arrest systems, nor did the employer demonstrate that it is infeasible or creates a greater hazard to use these systems and develop and implement a fall protection plan which meets the requirements of paragraph (k) of section 1926.502:    (a)   On or about August 11, 2017 at the location 2718 Beltline Road, Sunnyvale, TX, employees applying shingles to a new construction residential building roof and were exposed fall hazards to the lower level.

29 CFR 1926.1053(b)(1): Portable ladders were used for access to an upper landing surface and the ladder side rails did not extend at least 3 feet above the upper landing surface to which the ladder was used to gain access:     On or about August 11, 2017 at the location 2718 Beltline Road, Sunnyvale, TX, employees using two    (2) 24-feet Werner extension ladders that were separated to make four (4) ladders.  The ladders did not extend 3-feeet above the landing of the roof and exposing employees to a fall hazard.      (a) On or about August 11, 2017 on the North side of the building, two 12-feet ladders did not extend 3-feet above the landing of the roof.      (b) On or about August 11, 2017 on the South side of the building, two 12-feet ladders did not extend 3-feet above the landing of the roof.
