29 CFR 1926.403(b)(1): Employer did not ensure that electrical equipment is free from recognized hazards that are likely to cause death or serious physical harm to employees:     a) On or about July 25, 2017; second floor; the employees were exposed to an electrical shock hazard by exposed energized conductors (electrical boxes) which had not been inspected to ensure that they had been de-energized prior to the remodeling and framing process of the residential extension.       In accordance with 29 CFR 1903.19(d), abatement certification is required for this violation (using the Certification of Corrective Action Worksheet), in addition, documentation demonstrating the abatement is complete must be included with your certification.  This documentation may include, but is not limited to evidence of the purchase or repair of the equipment, photographic or video evidence of abatement, or other written records.

29 CFR 1926.404(b)(1)(ii): On a construction site, where an assured equipment grounding program was not utilized, all 120-volt, single-phase, 15 and 20 ampere receptacle outlets which were not a part of the permanent wiring of the building or structure and which are in use by employees did not have approved ground fault circuit interrupers for personal protection     a) On or about July 25, 2017; Second floor; employees were operating electrical tools such as but not limited to saws that were not plugged into a ground fault circuit interrupter (GFCI) exposing employees to a potential shock hazard.         In accordance with 29 CFR 1903.19(d), abatement certification is required for this violation (using the Certification of Corrective Action Worksheet), in addition, documentation demonstrating the abatement is complete must be included with your certification. 29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b):     a) On or about July 25, 2017; Roof area residential construction; employees were observed working on the roof area, of the new extension of the residential home, without fall protection in place to protect them from falling an estimated 25-30 feet to ground level while installing the membrane.      b) On or about July 25, 2017; Second floor; employees were exposed a fall of 20 feet while working near open wall openings in which the top rail had not been installed.        In accordance with 29 CFR 1903.19(d), abatement certification is required for this violation (using the Certification of Corrective Action Worksheet), in addition, documentation demonstrating the abatement is complete must be included with your certification.
