29 CFR 1926.501(b)(4)(i): Each employee on walking/working surfaces was not protected from falling through holes (including skylights), more than six feet (1.8 m) above lower levels, by personal fall arrest systems, covers, or guardrail systems erected around such holes: a) On or about 8/10/2017, Digester complex: A 30 inch by 55 inch floor opening was used as a ladder access. The unused portion of the opening was not provided with a guardrail system. Employees exposed to falls of 17 feet.

29 CFR 1926.1052(c)(1)(i): Stairways having four or more risers or rising more than 30 inches (76 cm), whichever is less, were not equipped with at least one handrail: a) On or about 8/10/2017, Lime stabilization building: A stairway which was four foot wide consisting of 22 steps running between the first floor and the second floor were not provided with a handrail. Abatement certification is required for this item.
