29 CFR 1910.132(a): Protective equipment was not used when necessary whenever hazards capable of causing injury and impairment were encountered: A) Arclin USA, LLC, located at 6175 American Road, Toledo, OH 43612: On or about August 4, 2017, the employer did not ensure that the employee was wearing appropriate chemical resistant clothing while troubleshooting the vacuum system associated with the reactor # 2 in the Resin Plant. The employee was working in a close proximity to the strainer, which was leaking due to the damaged gasket. The employee was exposed to the hazardous chemical products such as, but not limited to, hot caustic solution.

29 CFR 1910.132(d)(1)(i): The employer did not select and have each affected employee use, the types of personal protective equipment that would protect the affected employee(s) from the hazards identified in the hazard assessment: A) Arclin USA, LLC, located at 6175 American Road, Toledo, OH 43612: On or about August 4, 2017, the employer did not select, and have each employee use, the types of PPE that would protect each employee from the hazards identified in the hazard assessment. The employee was not required to wear appropriate chemical resistant clothing while troubleshooting the vacuum system, associated with the reactor # 2 in the Resin Plant, and working in a close proximity to the strainer, which was leaking due to the damaged gasket. The employee was exposed to the hazardous chemical products such as, but not limited to, hot caustic solution.
