29 CFR 1910.212(a)(3)(ii): Point(s) of operation of machinery were not guarded to prevent employee(s) from having any part of their body in the danger zone(s) during operating cycle(s): Location: 110 Willenbrock Road, Oxford, CT 06478 The point of operation where hand fed aluminum wire was curled around a rotating pipe on the pipe threading machine was not equipped with a guard to prevent any employee's bodily part from entering the danger zone(s) during operating cycles(s). Consequently, on or about August 11, 2017, an employee sustained serious injuries after the employee's hand was pulled into the point of operation resulting in multiple finger amputations of the left hand.
